Here is a questionnaire template for you to use and make your own. I have a questionnaire that I send my couples and then go over it with them on the phone. You can make a copy and change however you'd like. This is the exact questionnaire that I use for most of my couples. Sometimes I add in special ceremonies and rental items such as arbors and chairs if I am talking to a couple that purchased a package.
